Lessons learned
I have seen the benefit of keeping a review and approval process and its effect on reducing or even eliminate the problems of code changes that are put into the production environment.
An item that could be improved was the writing and documentation of tests, because some information was considered implied, making it harder for other persons to reproduce them on the customer acceptance environment. The writing of documents must be done with a better sense of the target audience and their level of knowledge.
